Customize your own form to meet your needs!

Prices include printing your form to your specifications and as a result already include your school name and address if needed. We can even scan a drawing or your school logo to place on your forms to further customize them! There is never a charge for typesetting or artwork.

INSTRUCTIONS

1. Put the title of your form in the "Form Title" box. For example you can write "Detention Notice", if you are ordering a Detention Notice form.

2. If you are modifying one of our standard forms, please put the form number in the "Original Form #" box. If you are creating an entirely new form, you can skip this step.

3. Create a rough draft of the new form. There are two upload fields above, one for your form, one for your logo (if applicable). You can use these to attach your files to the order, or if you prefer you can fax a copy of the form to us at 1-877-431-1201 (just reference your order number on the fax).

*Logos generally need to be print quality, which is a resolution of 300dpi. You can find the resolution of your image by right clicking the file and choosing "properties". The details tab should display the horizontal and vertical resolution.

4. Please include your contact information (fax or email) with your order so that we can send you a proof approval. New forms will generally require this step unless the changes are very minor. Once your receive your proof, you can make additional changes to your form. You will need to approve the final changes before your form is printed.

5. Please call for RUSH orders! Our normal turn-around is 2-3 weeks on custom orders but we can work with tighter deadlines if necessary at no extra charge.
