Absent-Tardy Admit Slip

Love the simple design, but need cardboard to prevent run through to slips below.

This form works for our school system of keeping track of tardies. I like the size and layout. If I could changed anything I would add a box for "Half-Day", along with Absent and Tardy. Other than that I don't know how you could improve the form. EMR

This was the first time I had ordered from National School Forms. The forms are what we were looking for. I liked the option of having them padded & duplicate. Makes our process much easier when were busy writing passes for students.

Allows me to add all the pertinent information that a teacher would need to excuse or not excuse a student.

Great basic absent-tardy admit slip. We have been using this form for a few years mainly as a tardy pass into class. One copy goes with the student to class and the other goes to the attendance officer so that she has all the information to update attendance.

We go through these like hot cakes. It is nice to have one in the office and one with the teacher.
