The Bus Conduct Report #174 is designed to report disciplinary incidents occurring on the school bus. We've recently updated it to include vaping and prohibited paraphernalia items as requested by our customers. It is available with either 3 or 4 copies (has an additional copy for the Transportation Office). The 3 part form is also available with an additional Parents' copy in Spanish.
